Does the LCSO have a form or something for the public to fill out when they leave their houses for an extended period of time, like 1-2 week vacation etc...??

Something to notify the Sheriff that the property would be vacant during set dates. And if a deputy sees something odd/suspicious at the property, the form would be the authorization needed for the LCSO to check it out.

There are forms that can be picked up at the substation for people going on vacation. During the time designated on the form the house will be visited by the V.O.I.C.E. personnel. If anything is out of place or suspicions a deputy will be informend.

While on vacation get stop delivery of newspapers and mail (it would be good to have a trusted neighbor check and pick up things just in case). It is also a good idea to have a neighbor use your driveway to give the appearance that the house is being used. (If you do this be sure to indicate it on the form so that the V.O.I.C.E. people will know to expet this and treat it as "normal.")
